Refund and No Show / Cancellation
The provider of services works with each client to discuss treatment options and their objectives, and to thoroughly review likely outcomes, benefits, and risks associated with each treatment. In the event the service can not be provided, either due to disqualifying health history or inability to obtain iv access, no charge will be applied. However, once the iv has been initiated and infusion has been started, no refund will be issued since this is a medical service.
We want our evaluations and treatments to not be hurried or appear so.
We ask that our clients make every effort to arrive on time and if there is a reason to cancel or move an appointment, that it is done with more than 12 hours notice. If the client arrives more than 15 minutes late, it is possible that we ask the appointment to be rescheduled.
If cancellations are not done with more than 12 hour notice or the client doesn’t show at all, Hydrate & Heal IV and Wellness reserves the right to charge $50 for your missed appointment.
This includes appointments where our service provider is unable to access the property or is turned away.
If Hydrate & Heal Iv and Wellness must cancel an appointment, a new appointment will be scheduled without penalty to the client, subject to availability.
Client Payment Responsibility
Insurance and Payment: The Client acknowledges that Hydrate & Heal IV And Wellness does not receive payment or reimbursement from managed care plans, Medicare, Medicaid, or any third-party payor, including health insurance, for its services. The Client will be billed directly and is solely responsible for payment, irrespective of potential reimbursement from any third-party payer.
We only accept cash or cards. No Checks accepted.
Patient Bill of Rights
Per Florida Statute 381.026, Florida law requires that your healthcare provider or health care facility recognize your rights while you are receiving medical care and that you respect the health care provider's or health care facility's right to expect certain behavior on the part of patients. You may request a copy of the full text of this law from your health care provider or health care facility. A summary of your rights and responsibilities follows:
Patient Rights:
1. A patient has the right to be treated with courtesy and respect, with appreciation of his or her individual dignity, and with protection of his or her need for privacy.
2. A patient has the right to receive a prompt and reasonable response to questions and requests.
3. A patient has the right to know who is providing medical services and who is responsible for his or her care.
4. A patient has the right to know what patient support services are available, including if an interpreter is available if he or she does not speak English.
5. A patient has the right to know what rules and regulations apply to his or her conduct.
6. A patient has the right to be given by the health care provider information concerning diagnosis, planned course of treatment, alternatives, risks, and prognosis.
7. A patient has the right to refuse any treatment, except as otherwise provided by law.
8. A patient has the right to be given, upon request, full information and necessary counseling on the availability of known financial resources for his or her care.
9. A patient who is eligible for Medicare has the right to know, upon request and in advance of treatment, whether the health care provider or health care facility accepts the Medicare assignment rate.
10. A patient has the right to receive, upon request, prior to treatment, a reasonable estimate of charges for medical care.
11. A patient has the right to receive a copy of a reasonably clear and understandable, itemized bill and, upon request, to have the charges explained.
12. A patient has the right to impartial access to medical treatment or accommodations, regardless of race, national origin, religion, handicap, or source of payment.
13. A patient has the right to treatment for any emergency medical condition that will deteriorate from failure to provide treatment.
14. A patient has the right to know if medical treatment is for purposes of experimental research and to give his or her consent or refusal to participate in such experimental research.
15. A patient has the right to express grievances regarding any violation of his or her rights, as stated in Florida law, through the grievance procedure of the health care provider or health care facility which served him or her and to the appropriate state licensing agency.
Patient Responsibilities:
16. A patient is responsible for providing to the health care provider, to the best of his or her knowledge, accurate and complete information about present complaints, past illnesses, hospitalizations, medications, and other matters relating to his or her health.
17. A patient is responsible for reporting unexpected changes in his or her condition to the health care provider.
18. A patient is responsible for reporting to the health care provider whether he or she understands a planned course of action and what is expected of him or her.
19. A patient is responsible for following the treatment plan recommended by the health care provider.
20. A patient is responsible for keeping appointments and, when he or she is unable to do so for any reason, for notifying the health care provider or health care facility.
21. A patient is responsible for his or her actions if he or she refuses treatment or does not follow the health care provider's instructions.
22. A patient is responsible for assuring that the financial obligations of his or her health care are fulfilled as promptly as possible.
23. A patient is responsible for following health care facility rules and regulations affecting patient care and conduct.
Good Faith Estimate Notice
You have the right to receive a “Good Faith Estimate” explaining how much your medical and mental health care will cost. Under the No Surprise Act, health care providers must provide uninsured patients, or self-pay patients, an estimate of the expected charges for medical services, including psychotherapy services. You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency healthcare services, including psychotherapy services. The Good Faith Estimate will also include items or services reasonably expected to be provided along with the primary item(s) or service(s), even if the individual will receive the items and services from another provider or another facility.
You can ask your healthcare provider, and any other provider you choose, for a Good Faith Estimate for a scheduled or requested item or service. You could be charged more if complications or special circumstances occur. If this happens, and your bill is $400 or more for any provider or facility than your Good Faith Estimate for that provider or facility, federal law allows you to dispute the bill. Keep a copy of your Good Faith Estimate in a safe place or take pictures of it. You may need it if you are billed a higher amount.
The Good Faith Estimate is not a contract and does not require the uninsured (or self-pay) individual to obtain the items or services from any of the providers or facilities identified in the Good Faith Estimate.

If you are billed for more than this Good Faith Estimate, you may have the right to dispute the bill.
You may contact the health care provider or facility listed to let them know the billed charges are higher than the Good Faith Estimate. You can ask them to update the bill to match the Good Faith Estimate, ask to negotiate the bill, or ask if there is financial assistance available.
You may also start a dispute resolution process with the U.S. Department of Health and Human Services (HHS). If you choose to use the dispute resolution process, you must start the dispute process within 120 calendar days (about 4 months) of the date on the original bill.
If you dispute your bill, the provider or facility cannot move the bill for the disputed item or service into collection or threaten to do so, or if the bill has already moved into collection, the provider or facility has to cease collection efforts. The provider or facility must also suspend the accrual of any late fees on unpaid bill amounts until after the dispute resolution process has concluded. The provider or facility cannot take or threaten to take any retributive action against you for disputing your bill.
There is a $25 fee to use the dispute process. If the Selected Dispute Resolution (SDR) entity reviewing your dispute agrees with you, you will have to pay the price on this Good Faith Estimate, reduced by the $25 fee. If the SDR entity disagrees with you and agrees with the health care provider or facility, you will have to pay the higher amount.

The Client represents and warrants no expectation of such services from HHIW. Any engagement is predicated on the understanding that HHIW's personnel will not provide medical care beyond the scope of the wellness and hydration services offered.
Risk Acknowledgment: The Client acknowledges risks associated with intravenous (IV) hydration, vitamin/supplement administration, and pharmaceutical services. The Client assumes all risks related to these services, barring gross negligence or willful misconduct by HHIW.
Detailed Ingredient Understanding and Safety Acknowledgment: By agreeing to this section of the Terms of Service, you, the Client, confirm that you have been thoroughly informed of and understand the exact ingredients and respective dosages included in your treatment. You acknowledge that all information regarding the ingredients and their dosages is available on our website for review. You agree that you have accessed this information prior to treatment and understand their intended effects. Furthermore, you assert that there are no contraindications based on your current health status that you have not disclosed to HHIW in written form. By proceeding with the treatment, you affirm that it is safe for you to receive the specified ingredients and dosages, having disclosed all relevant health information to ensure the safety and efficacy of your treatment.
Client Representations and Warranties: As a client of HHIW, you confirm that you are not currently using illegal drugs or controlled substances. You acknowledge that any failure to disclose true and complete health conditions or medication usage may increase risks associated with the treatment provided. HHIW relies on the accuracy and completeness of the information you provide and bases its service provision on this information. You agree to indemnify and hold HHIW harmless from any and all claims, damages, or other legal actions arising from inaccuracies in the information you provide.
Disclaimer of Warranties: HHIW offers its services on an "as is" and "as available" basis, without any warranties, express, implied, or statutory. Consent for Use and Disclosure of Protected Health Information (PHI): The Client consents to HHIW’s use and disclosure of PHI for treatment, payment, and healthcare operations (TPO). The Client may review HHIW's Privacy Policy prior to signing this consent. This consent includes authorization for HHIW to communicate via phone, mail, or email for purposes related to TPO. The Client may revoke this consent in writing at any time, with the understanding that revocation does not affect prior reliance on the consent.
Eligibility: Our services are available only to individuals who are at least 18 years of age. If you are not 18 years old, you are not permitted to use HHIW’s services and should not provide us with any personal information.
Emergency Procedures: In case of an emergency, the Client is advised to call 911 or proceed to the nearest emergency room. Hydrate & Heal IV and Wellness is not responsible for emergency medical care after the client has completed the infusion and left the event/mobile premises.
Indemnification: You agree to defend, indemnify, and hold harmless Hydrate & Heal IV and Wellness and their affiliates, along with their respective shareholders, directors, officers, medical directors, employees, agents, successors, and assigns, from and against all claims, liabilities, damages, judgments, awards, losses, costs, expenses, or fees (including reasonable attorneys' fees) arising out of or relating to your use of our services, your violation of these Terms, or your conduct in connection with our services. This includes, without limitation, any claims alleging facts that if true would constitute a breach of these Terms by you, any content you provide, any misrepresentation or fraudulent acts conducted by you, and any activity under your account and user ID, including negligent or wrongful conduct.
We will provide notice to you of any such claim, suit, or proceeding. Hydrate & Heal IV and Wellness reserves the right, at your expense, to assume the exclusive defense and control of any matter for which you are required to indemnify us, and you agree to cooperate with our defense of these claims.
